Since its inception in 2002, the Council for Affordable Quality Healthcare (CAQH) Universal Credentialing Datasource has registered over 350 health plans and 500,000 practitioners in the Datasource. The following questionnaire was developed to raise awareness of this free online credentialing application and to collect feedback from CAQH users to help improve the process.
Respondents' personal and practice identity will remain confidential. We will share only blinded data with HASC members, public officials and media. As a respondent, you will receive a report on questionnaire responses.
Here are the guidelines to assist you in completing the questionnaire:
1) Enter your e-mail address in Question 1. This will be your ID, enabling you to work on the questionnaire and return to it later if you don't finish it in one sitting. Results of the study will also be sent to this e-mail address.
2) If you are not an MGMA national member, please tell us which association directed you to this questionnaire, using the space provided next to “membership number."
3) This Web-based poll consists of multiple web pages, each with multiple questions. To move forward, click the Next button at the bottom of each Web page. To back up to a previous web page, click the Back button on your Web browser.
4) Use your mouse to move between questions. Do not use the Enter key. Doing so will skip questions and move you to the next Web page.
5) If you need to leave the questionnaire before completing it, click the Save and exit button at the bottom of the Web page. You can then return to this Web address and re-enter your email address. All previously entered answers will be recalled.
6) Do not be concerned if the questionnaire skips some questions. Some questions may be “conditional” on previous answers.
7) If you do not know the exact answer to a question, please provide your best estimate.
